Privacy notice for Zombie Smiles

Who we are

St. Louis South Oral & Maxillofacial Surgery, the oral surgery practice of Dr. William A. Gray, with offices in South County, Festus and Creve Coeur. The privacy policy at stldentalimplants.com/privacy-policy covers the rest of our website. Where the two differ, this notice applies to these campaign pages.

What we collect

Only what you type into the consult request form, plus a few details about how you found the page:

• Your first and last name and mobile number
• Your email address, if you give one
• The office closest to you, the best time to call and the topic you picked
• Whether you checked the box agreeing to text messages
• Your A or B vote, if you voted
• The campaign tags in the link you followed, such as which ad or post sent you

If you send a request through a lead form inside Facebook or Instagram instead, the same fields are collected by Meta under its own privacy policy and passed to us. We handle them exactly as described in this notice.

Please don't put medical details in the form. We go over your health history privately, by phone or at the consult.

Your browser holds your vote and the campaign tags in temporary session storage until you close the tab, so they can travel with your request. They stay on your device unless you send the form.

What this page doesn't do

• No advertising pixels or tags from Meta, Google or anyone else
• No analytics or tracking scripts
• No cookies set by these pages
• No third-party fonts or video players. The fonts, images and film load from the same site as the page. The consult form is hosted by Jotform, our form provider, under a signed business associate agreement. It is the only third-party request these pages make, and it happens when the form loads

Our hosting provider keeps standard server logs, such as IP address, browser type and the time of each request, to run the site and protect it from abuse. If you tap Directions, Google Maps opens in a new tab and Google's privacy policy applies there.

How we use it

• To call you, and email you if you gave an address, about your consult request
• To text you about your request, only if you checked the box
• To schedule your consult at the office you prefer
• To see which parts of the campaign people respond to, using the campaign tags and votes in totals

Who we share it with

We don't sell or rent your information, and we don't share it with advertisers. We share it only with service providers that help us receive and answer requests, such as our form, phone and scheduling systems, under agreements that require them to protect it and use it only for us. We may also disclose information when the law requires it.

Health information

Once you become a patient, your health information is protected under HIPAA and handled according to our Notice of Privacy Practices. You can get a copy at any of our offices or by calling 314-842-4699.

How long we keep it

We keep consult requests as long as we need them to respond and follow up, and as long as the law or our record-keeping policies require. If you never become a patient, you can ask us to delete your request at any time.

• The form sends your information over an encrypted (HTTPS) connection.
• These pages are meant for adults and are not directed to children under 13.
